Nadir bir opistotonus nedeni; fumarik asidüri: Türkiye'den ilk olgu sunumu
Fumarik asidüri; otozomal çekinik kalıtılan, idrarla fazla miktarda fuma- rik asit atılımı ile belirgin, nadir görülen metabolik bir hastalıktır. Doğum öncesi dönemde polihidramniyoz, intrauterin büyüme geriliği, beyin ventriküllerinde genişleme ve beyin anomalileri görülmektedir. Büyüme ve gelişme geriliği, hipotoni, nöbet ve beyin atrofisi, fumaraz eksikliği olan hastaların ortak özellikleridir. Kraniyal görüntülemede polimikrogri ve ventriküllerde genişleme en sık rastlanan bulgulardır. Akraba evlilik- lerinin fazla olduğu ülkemizde otozomal çekinik hastalıkların görülme sıklığının fazla olması beklenir. Aralarında akraba evliliği olan ebeveyn- lerden doğmuş, 45 günlükken hiperamonyemi ve opistotonus nedeniyle hastanemize yönlendirilmiş bir hastada, metabolik hastalık düşünülerek yapılan idrar organik asit analiziyle fumarik asidüri tanısı konulmuş ve bu tanı radyolojik tetkiklerle desteklenmiştir. Ülkemizde daha önceden bildirilmiş fumarik asidüri olgusu bulunmadığı için sunmayı uygun bul- duk. (Türk Ped Arş 2014; 49: 74-6)
A rare cause of opistotonus; fumaric aciduria: The first case presentation in Turkey
Fumaric aciduria is a rare autosomal recessive metabolic disease which is characterized with excessive fumaric acid exretion in urine. In the prena- tal period, polyhydramniosis, intrauterine growth retardation, enlarged brain ventricles and brain anomalies are observed. Growth and deve- lopment failure, hypotonia, seizures and brain atrophy are the common characteristics of patients with fumarase deficiency. On cranial imaging, the most common findings include polymicrogyria and ventriculome- galy. In our country where consanguineous marriages are common, the incidences of autosomal recessive diseases are expected to be high. In a patient who was born from a consanguineous marriage and referred to our hospital at the age of 45 days because of hyperamonemia and opistotonus, a diagnosis of fumaric aciduria was made with organic acid analysis performed considering metabolic diseases and this diagnosis was supported with radiological investigations. We thought this case was worth presenting, since there was no case of fumaric aciduria reported before in our country. (Türk Ped Arş 2014; 49: 74-6)
