Sigaranın bronş mukozası üzerine etkileri
Akciğer kanseri oluşumunda ilk basamak olan skuamöz metaplazi gelişiminde sigara dumanına maruziyetin etkilerinin araştırdması.Prospektif olarak sigara içen ue içmeyen olgularda fiberoptik bronkoskopla elde edilen bronş mukozası biyop-si örneklerinde metaplazi indeksi formülünü kullanarak, skuamöz metaplazi gelişiminin incelenmesi, izmir Göğüs Hasta-lıkları ue Cerrahisi Eğitim Hastanesi 11-12. Seruis' lerinde yatarak izlenen, sigara içen 53 ve sigara içmeyen 10 olgu çalış-maya alındı. Sigara içen grupla içmeyen grup ue sigara içenlerin de sigaraya başlama yaşı, paket yılı, günlük içilen siga-ra ve klinik tanıları dikkate alınarak oluşturulan alt grupları metaplazi indeksine göre karşılaştırıldı. Metaplazi indeksi si-gara içenlerde 16.3 ± 2.4 (0-96.7), sigara içmeyenlerde 7.4 ± 1.6 (0-11.8) idi ve aralarında istatistiksel olarak anlamlı fark saptandı (p=0.002). Sigara içen grupta da en yüksek metaplazi indeksi günde iki paket ue üzerinde sigara içenlerde sap-tandı ue daha az sayıda sigara içen grupla aralarında istatistiksel olarak anlamlı fark vardı (p=0.008). Sigara içenlerde ve özellikle günlük sigara tüketimi iki paket ue üzerinde olanlarda, akciğer kanseri öncü basamağı olan skuamöz metaplazi yüksek oranda saptandı. Sigaranın bronş mukozasına zararlı etkisi bir kez daha objektif olarak gösterildi ve sigarayı bırak-tırma çalışmalarına önem verilmesi gerektiği vurgulandı.

Effects of Smoking on Bronchial Mucosa To evaluate the effects of exposure to cigarette smoke on developing squamous metaplasia which is the first step ofcarci-nogenesis in lung. A prospective investigation of the presence of squamous metaplasia on specimens of bronchial mucosa obtained by bronchofiberoscope in a group of smokers or non-smokers by using the formula of metaplasia index (MI). 53 smoker and 10 non-smoker patients who were admitted to Izmir Chest Disease and Surgery Training Hospital, llth and 12th clinical departments. Comparisons were made between the subgroups stratified basing on their smoking habbit, be-ginning y ear of smoking and clinical diagnoses. Metaplasia index was 16.3 ± 2.4 (0-96.7) in smoker while it was 7.4 ± 1.6 (0-11.8) in non-smokers and the difference was significant (p=0.002). The highest MI was obtained in the group of patients who smoked 20 or more cigarettes per day and this was significantly different than the group of patients who smoked less (p=0.008). The precursor lesion of lung cancer, squamous metaplasia was found in high proportion in smokers and especi-a"y in the group of patients who smoked 2 packets or more cigarettes per day. We want to emphasize the importance of quitment strategies of tobacco use while showing the objective harmful effect öf cigarette smoke on bronchial mucosa on-ce again.
