Endobronşiyal yerleşimli selim tümoral lezyonlarda cerrahi tedavi
Akciğerin selim tümörleri nadir görülmektedir. Rezeke edilmiş akciğer tümörleri içinde selim olanların oranı %1 olarak bildirilmiştir. Selim tümörler akciğer kaynaklı bütün hücre tiplerinden köken alabilir ve parankimal veya daha nadiren de endobronşiyal yerleşim gösterebilir. Endobronşiyal kitlenin distalinde tıkanma sonrası gelişen olaylar nedeniyle tabloya hakim olan kronik infeksiyon ile ayırıcı tanı yapmak ve kitle ortaya konduğunda da bronkojenik karsinom olmadığının gösterilmesi en önemli sorunlardır. 1993-1998 yılları arasında Yedikule Göğüs Cerrahi Merkezi'ne poliklinik başvurusunda bulunarak incelenen ve opere edilen endobronşiyal selim tümörlü 5 olguyu değerlendirdik. Olgularımızın 3'ü kadın 2'si erkek, ortalama yaşları 42.8 (30-60) idi. En sık şikayetleri tekrarlayan solunum yolları infeksiyonu idi. Yerleşim 4 olgumuzda sağda, 1 olgumuzda solda idi. Olguların tamamına kesin tanı ve tedavi amacıyla torakotomi uygulandı. Üç olgumuzda intrabronşiyal hamartom, birinde psödopolip, bir diğerinde ise endobronşiyal lipom saptandı. Uygulanan cerrahi girişimler ise; 3 olguda lobektomi, 1 olguda alt bilobektomi, 1 olguda segmentektomi idi. Postoperatif mortalite saptamadığımız olgularımızdan alt bilobektomi yapılanda nazotrakeal aspirasyonla gerileyen atelektazi gelişti. Endobronşiyal selim tümoral lezyonlar nadir görülürler ve bronşiyal tıkanmaya bağlı semptomlara yol açarlar. Tıkanıklığın gerisinde oluşan infeksiyona bağlı değişiklikler kalıcı olmadığı sürece parankim koruyucu rezeksiyonlar tercih edilmelidir.
Surgery of endobronchial benign tumors
Benign tumors of the lung are occasionally seen. The rate of benign tumors is reported to be 1% of all resected lung tumors. They may originate from all types of cells and show parenchymal or rarely endobronchial localisation. The most important problems are to make differantial diagnosis from chronic infection due to parenchymal changes distal to obstruction and when tumor has located, from bronchogenic carcinoma. We evaluated 5 patients with endobronchial benign tumors admitted to the hospital, examined and operated in our clinic between 1993-1998. Three were female, 2 male with a mean age of 42.8 (30-60). The most frequent complaint was recurrent respiratory tract infections. The location was on the right in 4 cases and on the left in 1. All patients underwent thoracotomy for both definite diagnosis and treatment. We detected intrabronchial hamartoma in three cases, pseudopolip in 1, endobronchial lipoma in the other. The types of resections undertaken were lobectomy in 3 cases, bilobectomy inferior in 1, and segmentectomy in the last one. We had no postoperative mortality and only one patient with bilobectomy inferior developed atelectasis and managed by nasotracheal aspirations. Endobronchial benign lesions are rarely seen and cause symptoms due to bronchial obstruction. Unless changes due to infection beyond obstruction are persistent, parenchyme-saving resections should be prefered.
