Bir pulmoner parankimal amiloidozis olgusu
Sistemik amiloidozis, çeşitli dokularda amiloid birikimi ile seyreden, diğer organları olduğu gibi akciğeri de tutabilen, etyolojisi bilinmeyen bir hastalıktır. Olgumuz, renal biyopsi ve akciğer biyopsisinde amiloidozis saptandığından sistemik amiloidozis olarak değerlendirildi. Olgunun, tekrarlayan artrit ve karın ağrıları öyküsü olması, anne ve babasının akraba oluşu, makülopapüler cilt lezyonlarının varlığı ve splenomegali saptanması nedeniyle, ailevi akdeniz ateşine bağlı sekonder amiloidozis olduğu düşünüldü. Olgumuzda, sağ akciğerin üst lobuna lokalize bir konsolidasyon ve orta lobda atelektazi mevcuttu. Bu radyolojik bulgular, sistemik amiloidozisin pulmoner tutulumunda yaygın olarak görülenlerden farklı olarak, üst loba lokalize olması, konsolidasyon formunda olması ve difüz tutulum olmaması nedeniyle özellik arz ediyordu.
A case of pulmonary parenchimal amyloidosis
Systemic amyloidosis is a disease with unknown etiology, associated with the amiloid deposition in various tissues. The lungs may be affected in systemic amyloidosis as the other organs. Because our case proven as amyloidosis by lung and renal biopsies we accepted him as systemic amyloidosis. The cause of amyloidosis was thought as familial mediterrenian fever as the history of the patient was positive for recurrent arthritis, abdominal pain, maculopapullar skin lesions and splenomegaly. His mother and father were relatives. Chest roentgenogram showed a consolidation on right upper lobe and middle lobe atelectasis. Because these radiological findings were localized to upper lobe and it was a consolidation rather than a diffuse form; according to these findings it was differing from the usual form of systemic amyloidosis.
