Akciğer kanserinde düşük ve yüksek doz sisplatin kemoterapisine bağlı akut kusma ve bulantının profilaksisinde ondansetron tablet ve deksametazon
Tüm sitotoksik ilaçlar, doz, veriliş şeması ve yoluna bağlı olarak bulantı ve kusmaya neden olabilirler. Bu çalışmada, dü-şük (70 mg/m2, grup 1) veya yüksek ( 100 mg/m2, grup 2) doz sisplatin içeren kemoterapi rejimi alan akciğer kanserli olgularda, düşük doz oral ondansetron ve deksametazon (IV) kombinasyonun akut bulantı ve kusmanın profılaksisindeki etkinliği ve emniyeti araştırıldı. Daha önce kemoterapi almamış 60 akciğer kanserli hasta çalışmaya dahil edildi. Grup l ' deki 32 hastaya, 103 kemoterapi siklusu uygulandı. Grup 2' deki 28 hastaya, 93 kemoterapi siklusu uygulandı. Tüm has-talara, 8 mg ondansetron tablet ve intravenöz yolla deksametazon 20 mg uygulandı. Sisplatin uygulamasını takip eden 24 saat boyunca kusma epizodları, bulantı ve yan etkiler değerlendirildi, ilk kemoterapi sikluslarında, 1. grupta hastaların %94'ünde (30/32), 2. grup hastaların %85'inde sıfır veya 1-2 kusma epizodu tespit edildLTekrarlanan kemoterapi siklusla-nnda, akut kusmanın kontrolü 1.grupta %82'ye (58/71), 2. Grupta %71 'e (46/65) geriledi.Bulantının kontrolü (bulantı yok veya hafif bulantı) grup l 'de hastaların %74 'de (76/103), grup 2'nin %78'inde(72/93) sağlandı.lki grup arasındaki etkin-lik farkı istatistiki olarak anlamlı değildi. Kombinasyon genellikle iyi tolere edildi.Başağnsı ,en sık rastlanan yan etkiydi. Sonuç olarak, düşük veya yüksek doz sisplatine bağlı akut kusmanın profilaksisinde, 8 mg oral ondansetron, 20 mg int-ravenöz deksametazon kombinasyonunun etkinliği arasında önemli fark yoktur.
Ondansetron tablet plus dexamethasone in the prophylaxis of acute emesis and nausea associated with low or high dose cisplatin-based chemotherapy in lung cancer
Ondansetron Tablet Plus Dexamethasone in The Prophylaxis of Acute Emesis And Nausea Associated With Low or High Dose Cisplatin-Based Chemotherapy in Lung Cancer Depending on the dose.the route and schedule of administration all cytotoxic drugs can induce nausea and vomiting. The efficacy and safety of low dose oral ondansetron plus dexamethasone (IV) as prophylactic therapy for acute nausea and emesis were evaluated in lung Cancer patients undergoing chemotherapy with low (70 mg/m2, group 1) or high (100 mg/m2 ,group 2) dose cisplatin containing regimen. 60 chemotherapy naive, hospitalised patients with lung cancer were included in this trial, 103 chemotherapy cycles were administered to the 32 patients in group 1. 93 chemotherapy cycles were administered to the 28 patients in group 2. Ali patients received 8 mg ondansetron tablet and intravenous dose of 20 mg dexamethasone. The number of emetic episodes, nausea and side effects were evaluated during the 24 hours following cisplatin administration. Ninety four percent of the patients (30/32) in group 1, and eighty five percent of patients (24/28) in group 2 experienced zero or 1-2 emetic episodes in the first cycle chemotherapies. Control of acute emesis over repeat cycles of chemotherapy decreased to 82% (58/71) and 71 % (46/65) respectively. Efficacy rates (no nausea and mild na-usea) in prophylaxis of nausea were 74% (76/103) in group 1, 78% (72/93) in group 2. The efficacy was not significantly different between the two groups. Ondansetron plus dexamethasone were well tolerated. Headache was the most com-monly reported side effect. In conclusion efficacy of a single, oral dose of 8 mg ondansetron plus intravenous dose of 20 mg dexamethasone is not significantly different in the prophylaxis of acute low or high dose cisplatin-induced emesis.
