Rezeksiyon uygulanmış büyük boyutlu primer mediastinal kitleli yedi olgunun analizi
Primer mediastinal kitleli olguların çoğunda tek başına tedaviyi sağlayan rezeksiyon, büyük damarsal yapılar ile trakea ve ana bronşlar gibi büyük hava yollarına bası yapan, yaşamı tehdit eden boyutlara ulaşmış tümörlerde öncelikle düşünülmesi gereken yöntemdir. Bu çalışmada, Ege Üniversitesi Tıp Fakültesi Göğüs Cerrahisi Anabilim Dalı'nda 1997- 2004 yılları arasında, primer mediasten tümör ve kisti nedeniyle rezeksiyon uygulanmış 35 olgu arasından, tümör büyük boyutu 12-25 cm arasında değişen 7 olgu retrospektif olarak incelendi. Tümü ön ve orta mediasten yerleşimli tümörlerde semptomlar 2-12 aydır sürmekteydi. Primer tümörün eksizyonuna ek olarak, olguların 4'ünde perikard, 3'ünde akciğer ve 2'sinde parsiyel vena kava superior rezeksiyonu yapılmıştı. Bu tümörler malign timoma, timik karsinom, timik teratom, non-Hodgkin lenfoma, immatur teratom, indifferan malign epitelyal tümör ve lipom idi. Sınırlı sayıda olgu içeren bu seriye göre, tümörün ulaştığı büyük boyutlar nedeniyle şiddetli solunumsal ve kardiyovasküler sistem semptomları olan hastalarda, öncelikle semptomların ortadan kaldırılması, daha sonra da kesin histopatolojik tanının konması açısından rezeksiyon göz önüne alınması gereken önemli bir tedavi seçeneğidir.
Analysis of seven patients with huge mediastinal masses treated by surgical resection
Surgical resection which is usually adequate in the treatment of most cases with primary mediastinal masses, should also be considered primarily in the patients with huge mediastinal masses that compress the vascular structures, major airways and that threat their lives. Between 1997-2004, among 35 cases of surgically resected primary mediastinal masses and cysts, seven patients with a tumor size of 12-25 cm are evaluated retrospectively. Tumors are localized in the anterior and the middle mediastinum and causing symptoms for 2-12 months. In addition to the excision of the primary tumor, four of the cases had underwent pericardial, three had pulmonary and two had partial vena cava superior resection. Histopathological types consisted of malignant thymoma, thymic carcinoma, thymic teratoma, non-Hodgkin lymphoma, immature teratoma, indifferent malignant epithelial tumor and lipoma. Surgical resection should be considered promptly for relieving the symptoms and verifying exact histopathological diagnosis for the patients who had severe cardiac and respiratory symptoms due to huge mediastinal masses, with acceptable morbidity and no mortality.
