SEKONDER İYİLEŞME İLE TEDAVİ EDİLEN BİR “APLAZİA KUTİS KONJENİTA” OLGUSU
Aplazia kutis konjenita nadir görülen hastalık grubudur. Sıklıkla izolelezyon olarak saptanır. Patofizyoloji intrauterin travma, enfeksiyonve teratojenler ile ilişkilendirilmiştir. Cilt altında kemik yokluğundakomplikasyon sıklığı artar. Bu olgu sunumunda, kemik ve duradefekti olmayan aplazia kutis konjenita tanılı hasta sunulmuştur.391 gebelik haftasında, 3280 gr doğduğu öğrenilen olgunun skalpte,verteks düzeyinde 3x4 cm’lik, kemik ve dura defektinin olmadığı tekbir lezyon gözlendi. Hastada başka bir anomali tespit edilmedi. BOSkaçağı gözlenmeyen olguda, günlük yara yeri pansuman takibi ilesekonder iyileşmeye bırakılması kararı alındı. Kemik, dura ve galeanınintakt olarak bulunması nedeniyle cerrahi tedavi planlanmadı.Klinik pratikte nadir görülen bu vakaların medikal ve cerrahi tedaviseçeneklerinin planlanması ve karşımıza çıkabilecek komplikasyonlarolgumuzda tartışılmıştır. Kemik ve dura defekti olmayan hastalardayara sekonder iyileşmeye bırakılabilir.
AN APLASIA CUTIS CONGENITA CASE FOLLOWED BY SECONDARY HEALING
Aplazia cutis congenita is a rare disease group. It is often detected as an isolated lesion. Pathophysiology has been associated with intrauterine trauma, infection and teratogens. The incidence of complications increase in the absence of bone under the skin. We presented a case of aplasia cutis congenita without bone and dura defect. The patient was born 3280 g at 391 gestational week. A single 3x4 cm lesion without bone and dura defect was observed in the scalp at the vertex level. No additional anomaly was observed. Since the bone, dura and galea were intact no surgical treatment was decided. There was no cerebrospinal fluid leakage from the lesion, and it was left to secondary healing with wound dressing daily. The medical and surgical treatment options of these rare cases in clinical practice and the complications that may be encountered are discussed in our case. In patients without bone and dura defect, the wound may be left to secondary healing.
