NADİR BİR MÜLLERİAN ANOMALİ: OHVIRA SENDROMU-MULTİDİSİPLİNER YAKLAŞIMIN ÖNEMİ

Nadir görülen bir müllerian anomali olan OHVIRA sendromu uterus didelfis + ipsilateral tıkalı bir hemivajina + ipsilateral renal ageneziden oluşur. Her iki vajina arasındaki septumun rezeksiyonu ile drenaj sağlanacağından kolaylıkla tedavi edilebilir. Geç başvuru veya tedavi durumunda ise retrograd menstruasyona sekonder endometriosis ve endometriumun biriken menstruel kana ve onun yarattığı basınca uzun süre maruz kalmasına bağlı subfertilite gibi bazı komplikasyonlar gelişebilir. Pyometra ve serviks kanseri bildirilmiş daha nadir komplikasyonlar arasındadır. Bu yazıda dismenore ve pelvik ağrı nedeniyle kliniğimize başvuran 18 yaşında bekar bir OHVIRA Sendromu olgusu sunulmuştur. Doğru tanı için disiplinler arası iletişimin önemi vurgulanmıştır.

RARE MULLARIAN ANOMALY: OHVIRA SYNDROME-THE IMPORTANCE OF MULTIDISCIPLINARY APPROACH

OHVIRA Syndrome is a rare Müllerian anomaly consisting of Uterus Didelfis+ipsilateral obstracted hemivagina+ipsilateral renal agenesis. Drainage of obstructed hemivagina and uterus can easily be achieved via resection of intervaginal septum. Pooled menstrual blood can lead to chronic irritation and pressure on endometrium, retrograde menstruation and endometriosis causing subfertility in case of delayed diagnosis and treatment. Rare complications reported are cervical cancer and pyometrium. In this report, a case of OHVIRA Syndrome in a 18-year-old virgin admitted to our clinic with dismenorrhea and pelvic pain is presented. Importance of interdiciplinary communication for timely true diagnosis is emphasized.
