Kaflı-orofaringeal havayolu ile laringeal maskenin karşılaştırılması
Çalışmamızda, kaflı-orofaringeal havayolu (KOFH) ile laringeal maske (LM) uygulamalarının hemodinami üzerine yaptıkları değişiklikleri ve yan etkilerini karşılaştırmalı olarak incelemeyi amaçladık. Çalışmada elektif cerrahi girişim planlanan ASA I ve II sınıflamasına giren, genel anestezi altında, kısa süreli operasyon geçirecek 30 olgu rastgele 2 gruba ayrıldı. I gruba (n=15) KOFH, II. gruba LM uygulandı. Uygulamanın ne kadar sürdüğü kaydedildi. Her iki grup olguda indüksiyondan hemen önce (kontrol), KOFH yada LM yerleştirilmesinin 1. ve 5. dakikalarında ve KOFH yada LM çekildikten hemen sonra ortalama arter basıncı, kalp atım hızı, periferik oksijen saturasyonu, endtidal CO2 basıncı ölçümleri yapıldı. Tüm olgularda peroperatif postoperatif olarak oluşan komplikasyonlar (laringospazın, mide distansiyonu, hipoksi, bulantı, kusma, aspirasyon, havalanma güçlüğü, aritmi) kaydedildi. Her iki grup arasında bu izlediğimiz parametreler açısından istatistiksel bir fark saptanmadı. Sonuç olarak; kısa süreli cerrahi operasyonlarda, trakeal entübasyon yapılamayan veya istenmeyen olgularda KOFH ve LM kullanılmasının güvenli bir solunum yolu sağlayabileceği kanısına varıldı.
Comparison of cuffed-oropharyngeal airway versus laryngeal mask airway
In this investigation our aim was to compare hemodynamic and side effects of cuffed oropharyngeal airway (COPA) versus laryngeal mask airway (LMA). In our study 30 ASA I and II, class patients planned for elective, short operation under general anesthesia were randomized into 2 groups. In group I (n=15) COPA and in group II (n=15) LMA were placed. Placement times were recorded. In both groups before induction of anesthesia (control), 1. and 5. minutes after insertion of COPA or LMA and just after removing COPA or LMA, mean arterial pressure, heart rate, peripheral oxygen saturation, end-tidal CO<sub>2</sub> measurements were recorded. In all patients, peroperative and postoperative complications (laryngospasm, gastric distantion, hypoxia, vomiting, aspiration, difficulty in patient manual respiration, arrhythmia) were noted. In both groups there were no statistically differences in all parameters. As a result; in short surgical procedures when tracheal entubation is not wanted or cannot be made, COPA or LMA can provide a safe airway.
